Athletic Medical Forms: Please note the information on the BOTTOM of the page about our electronic forms.
Everyone must login to ATS to complete the pre-participation packet. This includes recertification for the winter and spring season. It is VERY easy to complete on-line.
In ATS you will complete all physical paperwork except Section 6, the physician page. Print this page out and have your physician complete the form and upload it back into ATS.
All PIAA Section 6 Forms must have been completed after May 1st to be valid for the upcoming school year. (ex. May 1st, 2025 and after are valid for the 2025-2026 school year)
Every student has been assigned a login and password. To login and complete the student paperwork go to:

(Parents are NOT send ATS password information. If you need your students ATS password please refer to the email that was sent to their CRSD student email located in the Fall/Winter/Spring Informational Emails)
