Athletic Medical Forms: Please note the information on the BOTTOM of the page about our electronic forms.
Everyone must login to ATS to complete the pre-participation packet. This includes recertification for the winter and spring season. It is VERY easy to complete on-line.
In ATS you will complete all physical paperwork except Section 6, the physician page. Print this page out and have your physican complete the form and upload it back into ATS.
Every student has been assigned a login and password. To login and complete the student paperwork go to:

**PLEASE CHANGE YOUR PASSWORD THE FIRST TIME YOU LOGIN AND LET YOU ATHLETE KNOW THE NEW PASSWORD AS THIS WILL BE NEEDED TO LOGIN AT THE ATHLETIC TRAINING ROOM**
